How to Send Specimens
Not sure where to start? Follow the directions below.
Tissue Cases
1. Fill out a requisition form, including a copy of the patient’s insurance card.
2. Attach labels to each specimen bottle.
3. Place tissues in formalin bottles.
4. Place bottles in a biohazard bag.
5. Insert requisition form into the biohazard bag pocket.
6. Place bag in a shipping box or tube container.
OR
7. Ship the package using FedEx Priority Overnight Airbill.
For Slide Consults
1. Fill out a requisition form or write a cover letter, including a copy of the patient’s insurance card and the prior pathology report.
2. Place slides in (a) a slide container wrapped in bubble wrap or (b) a bubble-wrap-lined manila envelope.
3. Place wrapped slides and all documents in a FedEx Clinical Pak, using a FedEx Priority Overnight Airbill.
